Consent to personal data processing

What exactly you allow when you tick “I consent to the processing of my personal data” under a form on the FAVOR website. Consent is voluntary: you can’t send a request without it, but you can simply call instead.

Document version

Version of 22.10.2026

This is a working version of the consent text. The wording is still being reviewed by a lawyer.

Consent text

By ticking the consent box and sending a request on the FAVOR website, I voluntarily agree that sole proprietor (FOP) Iryna Vasylenko (hereinafter, FAVOR clinic) may process the personal data I entered in the form on the terms described below.

I understand that I can withdraw this consent at any time, and that withdrawal does not affect processing that took place before it.

What data the consent covers

Only what you entered in the form yourself, plus service data the form adds automatically.

  • Phone number.
  • Your name or how you’d like to be addressed, if you gave it.
  • When it’s convenient to talk and what the concern is, if you filled in these fields in the form on the home page.
  • The address of the page the request was sent from and the time it was sent.

What for

So that the FAVOR clinic can call you back, answer your questions and, if you wish, arrange a consultation, a doctor’s visit or hospitalization.

  • The data is not used for mailings or advertising.
  • The data is not sold or shared with advertising networks.

Who has access

Clinic staff who call back and handle inquiries, and the services the request passes through.

  • Binotel telephony, when the clinic calls you back.
  • The full list and terms are in our privacy policy.

How long consent is valid

Consent is valid for as long as the data is needed for the purpose you left it for, or until you withdraw it.

  • The site doesn’t store the request in its database: it only forwards it by email.

Your rights and how to withdraw consent

The rights of the data subject are set out in Article 8 of the Law of Ukraine “On Personal Data Protection.” You can find out what is stored about you, correct it, demand its deletion and file a complaint.

  • To withdraw consent, call the hotline at (067) 151-53-03.
  • A complaint can be filed with the Ukrainian Parliament Commissioner for Human Rights (ombudsman.gov.ua) or with a court.

How you give consent

Every form has a consent box. It is never pre-ticked: without your action, the request isn’t sent.

If you call rather than fill in a form, no separate consent on the site is needed. What happens to data during a call is described in the privacy policy.
